Hospice Care and Palliative Care Services Provided

Generally, we provide routine home care support, counseling, and respite care services. A special type of care will be provided depending on the individualized care plan of the patient. The patient, including the coordination with his or her family, will be assessed in order to identify various needs and other healthcare requirements.

Contingent on the care plan developed by the hospice team, the hospice patient may receive the following services:

General Overview of Services

We have a team of professionals who will provide the necessary patient care and support for our hospice care in Lake Forest, California.

Hospice Physician

Our hospice physicians are responsible for creating and maintaining the healthcare management of the patient’s plan of care. They ensure that patients meet their needs every day and receive the highest level of care possible. On a regular basis, they assess the evaluation and reports of the patient’s hospice team and provide the necessary action and changes in the care plan.

RN / LVN

An RN / LVN provides hands-on, direct care for the terminally ill patient. Whether it’s administering medications, attending to ongoing wound care, or properly documenting the vital signs of the patient, RNs and LVNs play a crucial role in ensuring that patients remain as pain-free and symptom-free as possible.

Medical Social Services

Medical social workers act as a support for patients and families in addressing the challenges of hospice and palliative care. They listen to problems and issues faced by the family and provide emotional support in the decision-making process. When needed, they can also guide families in terms of finance, funding, funeral planning, and more.

Chaplain Support

For patients receiving hospice and palliative care, a chaplain is responsible for planning, assessing, and caring for a patient’s spiritual needs throughout his or her final leg in life. A hospice chaplain is an expert spiritual care provider and counselor that helps patients and families find peace.

Hospice Aides

A home health aide helps patients in terms of their practical needs at home. The duty of an HHA becomes the eyes, ears, and hands of a hospice care team as they are the personal contact of the patients. Duties of a home health aide include personal care, assistance with toileting, light housekeeping, meal preparation, and more.

Volunteer Support

In hospice care organizations, hospice volunteers offer a big help in achieving quality patient care. Volunteers can accompany patients at home, especially when the family and friends of the patients are not able to attend to them. Hospice volunteers can provide assistance with personal errands, light household chores, arranging appointments and transportation, and more.

Physical, Occupational, Speech Therapies

Therapists have a unique role in a hospice team. They help terminally ill patients slow down the decline of their functional ability, preserving their sense of self-worth. A physical therapist helps in dealing with balance, positioning, transferring, and using safety equipment. An occupational therapist deals with preserving the independence of the patient and lessening the reliance of a patient on a caregiver. Lastly, a speech therapist provides support in swallowing techniques to prevent aspiration and improve communication skills.
